Newsletter – Sept 2014

Our guest speaker at the Septembers meeting was Lucia Urquart – Hospital Link Worker from Halton Carers Centre, who came along to tell us about her role and how you can make your life easier when visiting/in patient at the hospital.

Lucia works closely with hospital staff to identify carers of patients, patients who are carers and staff carers to ensure that carers are fully supported and involved in the patients discharge and to help prevent re admission of the patient. She can provide information and contact details of organisations that can help and support carers and the person they care for for any hospital visits or admissions. An example of this, was a members cared for was going in hospital for the day, Lucia informed PALS of this admission and when they arrived at the hospital, staff knew they were coming in, they were admitted immediately and where first on the list, this made it easier for both carer and cared for, as there was no hanging about.

If carer is in hospital and admission is planned, care may have already been arranged for person who is being cared for by another family member or temporary care package put in place by community care team. If care has not been pre-arranged care can be arranged for cared for person while carer is in hospital.
If admission is not planned and patient is admitted in an emergency and there is no care in place for the cared for person and this person may be at risk please contact social services duty team. You can get a carer card which tells ambulance team that you are a carer and what has been put in place for your cared for in case of emergency. You can get these cards from the Halton Carers Centre in either High Street, Runcorn or Widnes, (based in Oakmeadows building)
